By completing and submitting our measurement form you are stating that you agree with and intend to comply with all our terms and conditions which are found on our faq/information page.

If you do not include a color with your measurement form, your gown will be made in white!

All special instructions for your gown must be included with your measurement form or they will not be incorporated into the creation of your gown.
